Moving Your Practice

When you move your practice, you must supply a new practice address effective immediately. A lapse in availability (see the office availability standards elsewhere in this manual) will cause Priority Health to assign your patients elsewhere to ensure continuity of care.

1. 60 days or more before the change takes effect, complete and print the Provider Change Form (515KB PDF) or the Behavioral Health Provider Change Form (117KB DOC).
   
2. Fax or mail the form to Priority Health at the address/fax number on the form.
3. If you move to a location that has not received a site visit previously, Priority Health will need to schedule a site visit to your new location.
