Common Elements of Physician Participation Agreements

The agreement begins on the date the physician successfully completes credentialing. (Learn more about credentialing.)
The agreement extends for one-year periods and automatically renews for additional terms unless terminated.
Either party may cancel the agreement without cause upon 90 days written notice to the other party.
The agreement may be terminated upon 60 days written notice with cause as defined in the agreement. For Priority Health Medicaid, the agreement may be terminated immediately with cause.
Physicians will submit claims for covered services within 90 days from the date of service.
Physicians will allow Priority Health to conduct reasonable standard reviews of their medical and billing records.
