Common Elements of Physician Participation Agreements
- The agreement begins on the date the physician successfully completes credentialing. (Learn more about credentialing.)
- The agreement extends for one-year periods and automatically renews for additional terms unless terminated.
- Either party may cancel the agreement without cause upon 90 days written notice to the other party.
- The agreement may be terminated upon 60 days written notice with cause as defined in the agreement. For Priority Health Medicaid, the agreement may be terminated immediately with cause.
- Physicians should submit claims for covered services within 90 days from the date of service.
- Physicians will allow Priority Health to conduct reasonable standard reviews of their medical and billing records.
