Billing for Observation

Observation, with surgery:
When surgery is performed in conjunction with observation room services, using the revenue codes 761 or 769 will allow the claim to be paid at the surgery fee schedule rather than the observation case rate.

Observation without surgery:
Outpatient services that include observation without surgery should be billed with revenue codes 760 or 762. The observation revenue code should only be reported once on the claim.
