To make sure you're receiving all the services and screenings you need to stay healthy, we have created a Personal Health Record for you. It's an easy way for you to keep track of them.

Our computers will scan your records for:
Preventive Care Opportunities, which include:

• Physical exams
• Immunizations
• Cancer screenings

For example, if your records don't show a physical exam in the last two years, our computer adds a physical exam to your Preventive Care Action List.

Disease Management Care Opportunities, which include services and prescriptions (if you have prescription coverage) for:

• High blood pressure
• High cholesterol
• Heart disease
• Asthma
• Diabetes

If your records indicate you might have one of these conditions and have not received recommended follow-up care, our computers add that care to an Action List for you.
