HealthbyChoice Rewards Instructions

How to earn HealthbyChoiceSM Rewards

You'll qualify for your employer's rewards when you complete all of your actions on time!

Before you start

Create your online account. You'll need a username and password to log in to priorityhealth.com.
Learn how to create your online account.

Gather the following information so it's at your fingertips when you need it. You may want to call your doctor or other health care provider to get this information. If you don't have it today, no problem. You can always update your HealthQuotient® later.
  • Height
  • Weight
  • Waist measurement
  • Blood pressure level (and date taken)
  • Cholesterol levels (LDL, HDL, total cholesterol, and date taken)
  • Triglyceride level (and date taken)
  • Blood sugar level (and date taken)
  • Month and year of most recent exams (i.e., physical, vision, dental, prostate, breast, flu vaccine)

Completing the WebMD HealthQuotient health risk appraisal

  1. On your personal home page look under Hello for HealthbyChoice. Click Go now.
  2. You will go to the WebMD HealthManagerTM website. The first time you go, you'll need to check the box that says you have read the WebMD Terms and Conditions and Privacy Policy. Then click Submit.
  3. Click Get started near the top of the page to begin the HealthQuotient. Answer each question as accurately as possible.
  4. Once you see that HealthQuotient is 100% complete at the bottom of the last page, click Submit. You will see your score, a HealthQuotient Summary and your health risks. WARNING: If you don't click Submit, HealthQuotient isn't complete!
  5. Take a look at your score, the HealthQuotient summary and your health risks. You can print this for your records, or to compare it to future scores.
  6. You can click Check Status or Your status to see other actions that are required by your program.
  7. To return to your Priority Health personal home page, click the link next to your name at the top of the page.

You may have other required actions

  • See your primary care physician for a physical exam
    1. Call your doctor to set up an appointment for a date before the deadline. Do it early! Most of our doctors schedule physicals two or three months in advance.
    2. See your doctor before the deadline.
    3. Once Priority Health receives your doctor's claim for your physical, this commitment will be marked with the checkmark that indicates the action is completed. If you do not get credit for an exam, call Customer Service.
  • See your treating physician for your health condition
    This means that you need to schedule an appointment to check on an ongoing condition such as diabetes, depression, high blood pressure or asthma.
    1. See your primary care doctor or other treating doctor to check on your health condition before the deadline.
    2. If you see your doctor for chronic care at the same time as your physical exam, ask your doctor to let Priority Health know you received a checkup for your chronic health condition. That way, your visit will appear in our data as both a physical exam and a chronic condition checkup, and this action will be marked "completed." If you do not get credit for an exam, call Customer Service.


Completing additional actions

Your employer may require that you complete more actions to qualify for rewards. Check with your human resources or wellness representative for these actions, deadlines, and rewards.
