call Customer Service for help.
First, create your personal online account
You only need to create your account once.
- Go to the home page of www.priorityhealth.com.
- Click Register Now.
- Enter data as it's requested: your contract number (from your Priority
Health ID card), your date of birth, and the last four digits of your
Social Security number. Answer the rest of the questions. Remember your
username and password, because you will need them to return to
HealthbyChoice and complete your commitments.
- Click Go to Your Member Center to get to your personal
home page.
How to log in once you have an online account
If you've forgotten either your username, your password or both, click
Forgot Password? and enter your contract number from your
Priority Health member ID card and other information. You will be given your
username and asked to create a new password.
- Go to the home page of www.priorityhealth.com.
- Enter your username and password in the fields (boxes).
- Click the login button. Your personal home page will
appear.
ALWAYS LOG OUT when you are finished visiting your personal online
account to protect your personal health information. Simply click
Logout in the upper right corner.
How to meet Your HealthbyChoiceSM Rewards commitments
1. First, log in to www.priorityhealth.com with your username and password, then
follow the step-by-step instructions below to meet your commitments.
2. Complete your HealthQuotientTM assessment*
- On your personal home page ("Hello Yourname"), find the
HealthByChoice box at the top and click the Get Started button.
- On your status page, find the Your Actions list. Click Update Your HealthQuotient
Now.
- Answer each question as accurately as possible.
- Once complete, you will see your HealthQuotient health risk score. Be sure to print your free, private health risk report for your records by clicking the Print Report link in the left column. Remember, this report is completely private. Your answers are stored on the WebMD website, so neither Priority Health nor your employer know what you entered or what your results are.
3. Set your personal health goal(s)**
Only one goal is required to complete your commitment, but you can enter up
to three health goals.
- At the top of any page, click the Your Status tab. Or click Home in the green menu bar, then go to the HealthbyChoice box at the top of your personal home page and click the Get Started or View Status button, whichever one shows.
- On your HealthbyChoice Status page, find the Your Actions list. Click Set your
health goal(s) now.
- Select a health goal from the list. OR, select
Other and write in your own goal.
- Briefly describe how you will meet your goal.
- Set the state of your Progress from the drop-down
list.
- Click Set Goals to save your information.
- Click Back to Status Page to review the status of your
commitments.
Note: You can come back to your goal in a day, a few weeks or even months
later and click on the
Update progress link on your
HealthbyChoice status page. Then, change your progress from "Just
Started" to "In Progress," "Completed" or "On Hold."
4. Meet your additional commitments
Your employer may have additional commitments for you to meet before you
become eligible for their incentives. Check with your human resources team
or wellness contact for your specific commitments, deadlines, and rewards.
Examples of commitments you may see listed in Your Action Items:
Update your HealthQuotient
- On your personal home page, go to the HealthbyChoice box and
click View Status to see if this commitment applies to
you and what the deadline is.
- On Your Action List, click Update your assessment
now.
- Answer each question as accurately as possible.
- Once complete, you will see your updated HealthQuotient Summary.
- Click the Your Status tab (upper left corner under the Health
By Choice page title) to review the status of your commitments*.
Visit your primary care physician for a physical exam
- On your personal home page, go to the HealthbyChoice box and
click View Status to see if this commitment applies to
you and what the deadline is.
- Call your doctor to set up an appointment for a date before the
deadline. Do it early! Most of our doctors schedule physicals two or three
months in the future.
- See your doctor for a physical exam before the deadline shown in Your
Action List.
- Once Priority Health receives your doctor's claim for your physical,
this commitment will be marked with the "Completed" checkmark and removed
from your Action List.
See your doctor for your health condition
- On your personal home page, go to the HealthbyChoice box and
click View Status to see if this commitment applies to
you and what the deadline is.
- See your primary care doctor or other treating doctor to check on your
health condition before the deadline.
- If you see your doctor for this care at the same time as your physical
exam, ask your doctor to let Priority Health know you received a checkup
for your chronic health condition. That way your visit will appear in our
data as both a physical exam and a chronic condition checkup, and your
commitment will be marked "Completed."
*When you have just completed or updated your HealthQuotient, you will see
a clock icon next to it on the Your Commitments list, indicating that your
information is in processing. You will see the "Completed" check mark on the
next business day.
**Log in to your account, go to the
HealthbyChoice box on your
personal home page, and click
View Status. You'll see
Your
Actions List. Click on
Set your health goals now and
follow the steps above.
