Q: Why don't I see the "Start Now" button in the HealthbyChoice box? A: Only members in employer groups who have elected HealthbyChoice have access to the HealthbyChoice area. If you've just enrolled in your employer's HealthbyChoice plan, you will get access to HealthbyChoice after we receive your enrollment information and it is entered into our system. If you have been enrolled in the HealthbyChoice plan longer than a week and still cannot log in, contact Customer Service. Q: I've heard about HealthbyChoice and am interested in understanding my health risks, but my employer does not offer HealthbyChoice. What is available for me? A: Understanding potential risks is the first step to taking control of your health. Any Priority Health member can go to their personal account page and take an online health risk assessment called the HealthQuotient® at no cost or obligation. Once you log in to your member account, just click on Health & Wellness in the green menu bar, and then click "HealthQuotient." Q: Why does it show that I completed commitments late when I am a new enrollee? A: The annual commitment dates are set each year, but don't worry. As a new enrollee you have 30 days to complete your HealthQuotient and set your health goal. Contact your Human Resources representative if you have questions. Q: I am new to Priority Health and have had a physical in the last two years. Do I need another one? A: No. Contact Priority Health Customer Service. They can determine if you had the correct procedures to count as a physical exam and update your commitment status. They will need you to give them the date of your last physical exam. Return to top Q: I completed my HealthQuotient and set a health goal, but my status is not showing complete. Why? A: On your status page, you should see a clock icon stating that it is processing. Our system updates commitment status overnight, so tomorrow it should indicate your commitments were completed. If you don't see the clock icon:
Q: I missed the due date for completing my HealthQuotient. Can I still take the health risk assessment? A: Yes, you can take the health risk assessment at any time. (Of course, your HealthbyChoice status page will not show the commitment as being completed on time.) To access the HealthQuotient if the link is no longer available from your HealthbyChoice status page, first log in to your online account here at priorityhealth.com. Then click Health & Wellness in the top green menu bar and choose "HealthQuotient." Q: I completed my physical exam commitment. Why isn't it showing up on the HealthbyChoice status page? A: Physical exams are entered in the system when doctors bill Priority Health for them. It may take a few weeks to receive the information from your physician's office. If you click the View Claims summary on your member account page and see that we have received the claim from your doctor's office, but your HealthbyChoice status page still is not showing that you met your physical exam requirement, contact Customer Service. They can determine if you had the correct procedures to count as a physical exam and update your commitment status. Q: My "Your Actions" page says I should be involved in a disease management program, but I don't believe this is correct. What can I do? A: Contact Customer Service. They have a process for reviewing the case and updating the commitment if it doesn't apply to you. Q: I logged in in the past but I cannot remember my username or my password. How can I get my username and password? A: On the priorityhealth.com home page, click "Forgot your password?" The system will ask you for your Priority Health contract number, and then tell you your username and ask you to create a new password. Q: The system says my account has been deactivated. How do I reactivate my account? A: On the priorityhealth.com home page, click on "Forgot your password?" and enter your Priority Health contract number. The system will walk you through reactivating your account. Q: Can I see what my HealthbyChoice commitment status was last year? A: Yes. You can see multiple years' worth of history. Select the time period you'd like to see by clicking at the bottom of the Commitment Details section of HealthbyChoice screen. Q: I have HealthbyChoice requirements from two employer groups. How do I make sure that I have completed the appropriate commitments? A: You can see what commitments you need to meet for each separate employer when you select the employer at the top of your HealthbyChoice screen. Deadlines for your commitments will vary by employer group, so make sure that you check both Action Lists to make sure that you are qualifying under both groups. Return to top
