Print these easy step-by-step instructions for creating your HBC account and completing your HealthbyChoice commitments.

You'll need your Priority Health insurance contract number from your ID card to create an account.


Your employer offers you HealthbyChoice RewardsSM tools and information to help you make smarter health care decisions. The benefits to you when you use these program tools are:
  • Increased awareness of your health risks
  • Information and support to make healthy lifestyle changes and manage your health
  • A healthier you

First, you need to register your account online
HealthbyChoice Rewards is an internet-based program. You'll need to create a username and password so you can get to your personal, private account information on this website - your claims, prescriptions, and more. Once you register your priorityhealth.com account, you can check it anytime. You'll see your copayments and claims, your deductible and other balances, and your HealthbyChoice status - which commitments are complete, and what your deadlines are.

Completing your HealthbyChoice commitments
This program encourages you to take specific actions by a specific time, called "commitments," to improve your health. When you participate in HealthbyChoice as part of your health insurance, your commitments might include:
  • Taking a health risk assessment called HealthQuotientTM
  • Setting a personal health goal, such as "Eat another serving of green vegetables each day."
  • Getting a physical
  • Other health commitments your employer sets.
