About the Rewards Program

Your employer offers you HealthbyChoice Rewards^SM tools and information to help you make smarter health care decisions. The benefits to you are:

First, you need to register your account online

HealthbyChoice Rewards is an internet-based program. You'll need to create a username and password so you can get to your personal Priority Health account and home page. Once you create your account, you can check it anytime. You'll see your copayments and claims, your deductible and other balances, and a link to the WebMD HealthQuotient®.

Completing your HealthbyChoice actions

This program encourages you to take specific actions by a specific deadline to improve your health. When you participate in HealthbyChoice as part of your health coverage, those actions might include:

• Taking the WebMD HealthQuotient online health risk appraisal
• Getting a physical exam
• Other required actions your employer sets.
