You must meet the requirements of the
HealthbyChoice Incentives
SM plan in the first 90 days of every plan year.
If this is your first year in the plan:
- If you and your spouse are both covered by the HealthbyChoice Incentives plan and you both meet the
Choice level requirements in the first 90 days, you remain at the
Choice benefit level through the end of the plan year. This includes
your covered dependents as well. Continue using your existing member ID
cards. You'll remain on the Choice benefit level the first 90
days of the next plan year too. During those 90 days you'll have the
opportunity to requalify for the Choice benefit level.
- If you don't meet the Choice level requirements, you are
automatically transferred to the Standard level of benefits for
the remainder of the year. You receive the same benefits but pay more for
copays, deductibles/out-of-pocket costs and coinsurance. Priority
Health will send you new ID cards.
For renewing HealthbyChoice Incentives members:
- If you are already at the Choice level of benefits, you stay at the Choice level for the first 90
days of the new plan year. You must meet the requirements again during those 90 days to maintain the
Choice level of benefits the rest of the plan year. You'll keep your ID cards.
- If you are already at the Standard level of benefits, you remain at the Standard level for the first 90
days of the new plan year. You must requalify during those 90 days. If you do not meet the requirements for the
Choice level of benefits, you'll remain on the Standard level
of benefits the rest of the year. You can continue using your existing member ID cards.
Remember, you always have the opportunity to qualify for the Choice
benefit level the next plan year.
- If you were on the Standard level of benefits for the first 90
days of the plan year and you meet the requirements to move up to the
Choice level of benefits, you'll receive new member ID cards from
Priority Health. Discard your old cards and enjoy the money-saving
Choice level benefits.
