HealthbyChoice Incentives renewal

During the first 90 days of the new plan year, you'll stay on the same benefit level (Choice or Standard) you were on at the end of the previous plan year.  Unless your employer has changed plan designs (a different HMO or PPO option), you'll use the same ID cards for the first 90 days of the new plan year.

• You'll have 90 days to qualify for, or re-qualify for, the Choice level of benefits.
  
• You (and your spouse, if covered under the plan) need to fulfill the two requirements each plan year to qualify for the Choice level of benefits.
• If you're benefit level changes after the 90-day qualification period (Choice to Standard or Standard to Choice), you'll receive new member ID cards.
