Q: Can I use results from a worksite biometric screening to meet the health indicator criteria on the qualification form?
A.  No. The health indicators must be measured and reported by your doctor, and submitted on the qualification form.

Q: Why are spouses required to participate?
A: If a member's spouse is covered by the HealthbyChoice Incentives plan, both must meet the requirements to maintain the Choice level of benefits. Studies have shown that couples working together toward a common goal have a greater likelihood of achieving successful outcomes.

Q: Why were the three specific health indicators selected?
A: The health indicators represent controllable health behaviors that are linked to chronic illnesses and other health issues. The U.S. Centers for Disease Control and Prevention estimate that 50 percent of today's health care costs are attributable to health risks that we can change through lifestyle behaviors such as diet and activity.

Q: Why do the requirements have to be met annually?
A: The plan encourages members to continue making healthier lifestyle choices by requiring them to meet the requirements for the Choice level of benefits every year that they are covered by the plan. This helps members be more aware of their health and helps to identify potential health risks that can be corrected before they become more serious.
