Q: What information is included in my online personal account with Priority Health?

A: Your online account shows your personal account information:

• Your claims and whether or not we have paid your doctor
• Your prescription history (if you have prescription coverage)
  
• Your "Personal Health Record," which shows checkups or screenings that might be due for you
  

Q: What can I do online using my account?

A: Your online account lets you:

• Change your Primary Care Physician (PCP) online
  

• Order replacement or additional ID cards online
  
• Use the Health & Wellness section of our site to:

• Use HealthQuotient^TM, a tool to help you identify and analyze your personal health risks
• Search for health care, disease and drug information from WebMD, a database of health information
• Learn about improving your fitness and starting a self-paced exercise plan
• Get help to quit smoking
• Plan a pregnancy
• Create a healthy diet
• Track your progress as you use any of these programs

Q: How do I get to see my online account?
A: It's easy. Go to our login page and click "Register now." You will need:

• Your account number from your ID card (every family member has his or her own contract number)
  
• Your date of birth
• The last four numbers of your Social Security number.
