Notify your company/HR when you change address.

Notify our Customer Service for changes in:
  • Marital status
  • Place of work
  • Dependents (a student over 19, add a new dependent etc.)
  • Insurance coverage
If you're covered by a PriorityHMOSM, PriorityEPOSM, PriorityPOSSM, or PriorityPPOSM health plan, this handbook will help you find out how to get health care, who to call, and what your rights are. 

What is covered?
To find out if your plan covers specific services, call Customer Service or see your coverage documents - either a Certificate of Coverage booklet or your insurance policy. Standard documents are available here online, but your employer's plan may differ.
