Make your Wishes Known

You have the right to make decisions about your medical care. You have a right to accept or refuse medical or surgical treatment for you or your child. You also have the right to plan and decide the types of health care you get in the future if you become unable to express your wishes.

Let your doctor know about your feelings by making a "Living Will" or "Power of Attorney." Call your doctor's office or our Customer Service Department at 1 888 975-8102 for a copy of the form.
