Have you just learned you have diabetes? Do you want more information about how to prevent diabetes, or care for someone who has it? Members of the Priority Health Diabetes Program receive postcards from our health management team, reminding them of important ways to stay their healthiest.

Talk with a nurse case manager

If you need extra help - perhaps because you are dealing with additional health risks - a case manager will help you:

  • Manage your diabetes, eye disease, kidney disease or other diabetes risks
  • Send you educational materials
  • Work with your doctor if necessary
  • Answer questions and offer support
