Talk with a nurse case manager
If you need extra help - perhaps because you are dealing with additional health risks - a case manager will help you:
- Manage your diabetes, eye disease, kidney disease or other diabetes risks
- Send you educational materials
- Work with your doctor if necessary
- Answer questions and offer support
