Your Personal Health Record
When you're a member of a Priority Health health plan and you log in to your account online, you'll see your Personal Health Record. It will show you an "Action List" of services and screenings you might need to stay healthy.
This information gets here because our computers scan your records and information your doctor enters and for the kinds of medications you are taking, looking for any care you may need.
Your preventive care opportunities
The Personal Health Record keeps track of preventive care you may need based on recommendations in the Preventive Health Care Guidelines. If you haven't had a physical exam in more than 2 years, for example, our computers will automatically add it to your Action List. Preventive care includes:
- Physical exams
- Immunizations
- Cancer screenings, like Pap tests and colonoscopies
Your chronic disease care opportunities
If your records or prescriptions show that you might have one of the conditions shown below, you'll see a separate Action List for that condition. If you are taking blood pressure medicine but you have not received follow-up care for it, our computers automatically add that care to an Action List for you.
- High blood pressure
- High cholesterol
- Heart disease
- Asthma
- Diabetes
