Forgot your username or password?
No problem.
- On the home page of priorityhealth.com, click Forgot username or password?
- Enter your contract number from your Priority Health member ID card.
- Enter and the last few digits of your Social Security or Medicare Claim number and your date of birth in the boxes.
- Our system will give you your account username.
- Enter a new password. (We don't have access to your old password, so we can't tell you what it was.)
