Your Priority Health ID cards

Your ID card helps your doctor and other health care providers know how to bill for services. It includes:

  • Your name
  • Your Priority Health contract number
  • Your group number (if you're a member of an employer's health plan)
  • Provider network information
  • Dependents (spouse, children) covered by your plan
  • Priority Health's billing address
  • Priority Health Customer Service numbers
  • What network to call when you're traveling to other parts of the U.S.

ID cards are mailed directly to your home. You should receive your ID card within 7-10 days from the date your information was sent to Priority Health.

Corrections to ID cards

When you receive your card, check it carefully to make sure all information is correct. Contact Customer Service with changes or revisions.

When to show your ID card and why

You should show your ID card whenever you visit a:

  • Doctor's office
  • Hospital or emergency room
  • Clinical laboratory
  • Pharmacy

The ID card helps your doctor's office or other provider know how to bill you and/or Priority Health for services:

In-network services save you money: In-network health care providers have a contract with your Priority Health plan and they give us a discount on your health care services. When you have to meet a deductible and are paying full price for services, you get this discount.

Out-of-network services usually cost more: Health care providers who are not under contract with Priority Health set their own. When you go to them, you may need to pay in full at the time you receive their services. If your plan covers services you get from out-of-network health care providers, you will then need to send Priority Health a request for us to pay you back.
