Formulary Changes: Diabetes Supplies

Effective August 1, 2007, Priority Health will no longer cover Ascensia®, Freestyle®, and BD® blood glucose meters and testing supplies as approved (formulary) drugs.  ACCU-CHEK®, OneTouch®, and TrueTrack® blood glucose meters and testing supplies will remain on the formulary.

Your employees who filled a prescription in the last year for Ascensia®, Freestyle®, and BD® blood glucose meters and testing supplies are being notified of this change by letter.  The letter encourages them to contact their provider for a new prescription for a glucometer and test strips. 

Strips Covered by Generic Copayment
TrueTrack® Test Strips Use the coupon enclosed with the test strips to get a free TrueTrack meter	TrueTrack strips may also be sold under store names, such as Meijer, CVS, or Walgreens
Strips Covered by Brand Copayment
ACCU-CHEK® Test Strips Call 877 234-6198 to get a free ACCU-CHEK meter and 50 free test strips	Compact Plus System, Aviva System
OneTouch® Call 888 883-4252 to get a  free OneTouch® meter and 10 free test strips	Ultra® 2 System, UltraSmart® System, UltraMiniSM System

Note:  If you have a three-tier prescription plan, the following strips will be covered at your third-tier (highest) copayment.

How Employees Can Get a Free Meter and Test Strips
Your employees can call the toll-free numbers in the table above.  We are communicating this opportunity to your employees as well as to their health care providers. 

• A prescription is not needed to obtain a free ACCU-CHEK or OneTouch meter.
• A prescription is needed to obtain a free TrueTrack meter. 
• A prescription is needed for test strips and lancets for all products.

The free meter is offered and paid for by the manufacturer, providing additional cost savings to you and your employee.  A copayment will apply to testing supplies.
