Quoting large groups (51+ employees)

Here's how to request a large group quote from Priority Health:

1. Complete and submit an Agent Agreement (600KB PDF), if you haven't already.
2. Have certain information ready when you call. Jump down to information for:
   
   • All groups with 51+ eligible employees
   • Groups with 100+ eligible employees only
3. Contact your region's sales representatives to request a large group proposal.
   
   • West or North Region: Contact your sales representative directly. Find their phone number or e-mail address.
   • East Region: Send an e-mail to fh-largegroupsales@priorityhealth.com

We'll need some basic information about the group as well as their current health plan information to create a quote.
