Employer-driven wellness program

Employer defines customized health requirements, due dates and types of rewards
Financial incentive for members is the better of two benefit levels (Choice vs. Standard). The Choice level features lower copays, deductibles, out-of-pocket costs and coinsurance.

Members and their families achieve the Choice benefit level if they meet two requirements set by Priority Health within 90 days of their effective date of coverage.
Employer rewards members based on their individual timelines Priority Health offers incentive upfront through the opportunity to maintain the Choice benefit level for the plan year – and into the first 90 days of the next plan year – if specific requirements are met within 90 days of effective date of coverage. 
Member does not have to meet any specific target health criteria to collect rewards 
To maintain the Choice benefit level, employees and their spouses must complete a qualification form indicating the status of three health indicators.

If members don't meet this requirement, they may also maintain the Choice benefit level by completing alternative requirements.
