How to choose a primary cary physician
Our online Find a Doctor directory is easy to use. You can find your own current doctor or look for a new one. Here's how:
Already have a doctor?
To check if your doctor is in your Priority Health plan network, you'll:
- Choose your plan (check your ID card or plan documents) and enter your location. Click Next.
- Enter the first few letters of your doctor's last name in the box on the right. Click Search.
- You'll get a list of doctors' names that match those letters. Click your doctor's name to see details, including address, hours, ID number, and accepted Priority Health plans.
- After you've confirmed you and your family members' doctors are in our network, enter the doctors' information on the PCP selection form. Open the PCP selection form.
Need a new doctor?
- Choose your plan (check your ID card or plan documents) and enter your location. Click Next.
- Choose a provider type (such as PCP, specialist, hospital, etc.) from the list on the left. You can pick a specialty (such as family practice, pediatrics, etc.) to narrow your results. Click Search.
- You'll get a list of names that match the type of provider you chose.
- To narrow the list down, click Filter results and check the options that are important to you. The list will automatically update with every option you pick.
- Write down the first and last name of the PCP you choose for each member of your family.
- Enter your choices on the PCP selection form. Open the PCP selection form.
