Behavioral health information for Priority Health employees

When requesting behavioral health information on a Priority Health employee, please call our Behavioral Health department at 800 673-8043 and ask to speak with a clinical case manager. If you need to leave a confidential message for a clinical case manager, please include only the member’s contract number, but not the member’s name. In addition, refrain from faxing forms or leaving messages regarding services for Priority Health employees and/or their families.

Submission of claims for Priority Health employees

Please submit Priority Health employee claims separately. Priority Health employees have the Group Number 496100 on their ID cards. Address claims to:
