Follow-up PCP Visits Post-Hospital Discharge measure

The Follow-up PCP Visits Post-Hospital Discharge measure provides an incentive when members receive care with their primary care provider within seven days of inpatient discharge when the admit type was emergent, urgent or trauma. Year-end payment of this measure is attributed to the PCP to whom the member was assigned on the date of discharge, not the member’s PCP on December 31, 2011. This measure includes members with HMO/POS or Medicaid coverage.
