Coordination of care
2010 Exchange of information survey results
Priority Health conducted a survey of PCPs that focused on the exchange of information with Behavioral Health (BH) specialists. The goal of the survey was to identify areas of opportunity to promote coordination of care and support quality improvement. The survey measured the frequency with which PCPs perceive that BH specialists share reliable and sufficient clinical information.
Results from this survey indicate that 51% of PCPs feel they receive relevant clinical information from BH specialists. PCPs involved in the survey said that for successful coordination of care, they would like:
- Detailed information regarding current treatment episode including medications, symptoms, treatment plan and anticipated length of treatment
- Advisement of changes in medications and treatment plans
- Notification of discharge when treatment concludes
