Reminder calls to your Medicare patients from a Priority Health nurse
A Priority Health nurse will make calls to your patients on the following HEDIS topics:
- Female patients who suffered a fracture. Patients will be encouraged to complete a bone
mineral density test or ask their doctor for a prescription for a drug to treat or prevent
osteoporosis in the six months after their fracture.
- Patients with a new diagnosis or newly active COPD will be encouraged to complete a
spirometry test to confirm the diagnosis.
- Patients with diabetes who are overdue for a retinal exam, A1c or LDL-C screening will
be encouraged to complete these screenings.
