Non-adherent member exclusion requests

A provider may request exclusion of a member for the 2011 program year at any point prior to Dec. 31, 2011. We require three separate and identifiable member outreach efforts. Priority Health will review each request and provide a determination. We’ll notify you if we need more documentation.
