NCQA payment for recognized practices
Priority Health will reimburse practices achieving PPC-PCMH NCQA recognition a PMPM infrastructure support payment of $1-$3 (based on tier 1-3). PMPM payment is based on the practice’s fully funded Priority Health members. This enhanced reimbursement will continue for up to a total of 12 months, based on the level of
recognition.
To be eligible for these payments: - You must submit your completed application to NCQA on or before Oct. 31, 2010.
- You must forward the NCQA application confirmation e-mail to your provider account representative as evidence of your submission.
- Once you receive confirmation that NCQA recognizes your practice as PPC-PCMH, forward the notification e-mail to your provider account representative.
