Spring PCMH Learning Collaboratives draws 200
Priority Health hosted the spring Patient-Centered Medical Home Learning Collaboratives April 20 and 21 in Grand Rapids. The two-day event drew 200 professionals from our PCMH pilot practices engaged in transforming their practices into medical homes. Speakers discussed topics including huddles, the "Teamlet model", open access, patient engagement and the Triple Aim.
Learning Collaboratives are an opportunity for professionals to network with colleagues from other practices and share the challenges and successes of the medical home transformation. They present another forum in which Priority Health shows its continual support for primary care.
Representatives from our pilot practices will be sharing their knowledge at our upcoming Priority Health Academy being held in the fall. More information will be included in the next edition of this newsletter.
