What can the 2010 Alliance for Immunizations (AIM) toolkit do for you?

  • A guide for mandatory physician disease reporting — Physicians are required to report occurrences of evident diseases to local health departments. Every disease prevented by routine immunization is reportable. Review the flyer Reportable Diseases in Michigan in your AIM kit or visit aimtoolkit.org.
  • Sources of vaccine information and CME opportunities — Participate in the Vaccines for Children Program so your practice can receive vaccines at no charge for children up to 18 years who are Medicaid eligible.
  • Tips for talking to families — This section includes Common Questions Parents Ask, Immunization & Pregnancy and a list of transportation services for Medicaid patients.
  • Provides sample forms for the office — Standing Orders for Influenza/Pneumococcal Vaccine; Healthcare Personnel Vaccination; Administration Records for Children, Teens and Adults; a Refusal Form; Vaccine Storage and copies of the Vaccine Information Statement (VIS). (Federal law requires VIS to be given to all patients receiving vaccines.) Visit immunize.org for VIS in different languages.
  • Information on MCIR — The Michigan Care Improvement Registry (MCIR) is a lifespan immunization registry for providers to report adult and child immunizations. Note: MCIR data is uploaded into Priority Health's Patient Profile monthly. Log in at priorityhealth.com.
