Sharing patient information with Priority Health

At times you’ll receive a request from us for documentation included in a patient’s medical chart. You are required per your contract with Priority Health to release the information to us at no charge. If you receive a phone call request and aren’t familiar with the caller, please ask the caller to fax his or her request to you on our letterhead, or you may contact your PAR/provider account coordinator (PAC) to confirm the identity of the caller. We appreciate your cooperation.
