PCP records transfer requests

When a patient requests that his or her records be transferred to a new PCP, Priority Health must be made aware of this change. We don’t always receive notice from the member, so please remind patients to contact us when they choose a new PCP. Log in to the Eligibility screen of the Provider Center to verify a member’s PCP.

If a member hasn’t contacted us with a PCP change, we’ll accept a copy of a records transfer request form from your office if it is received within six months of the date of the member’s signature on the form. Incomplete forms will result in a delay in updating our system. The form must include the new PCP's name, not the name of the provider group. You may fax these requests to 616 975-8828 or email them to B&E.customerservice@priorityhealth.com. If you’re mailing the request, please include MS1105 in the address.

Please note that if the member is enrolled in a product that doesn’t require a PCP assignment (such as a PPO), you don’t need to notify us. You can verify product information by logging in to the eligibility application in the Provider Center.
