Non-adherent member exclusion requests

A provider may request exclusion of a member at any point prior to Jan. 31, 2011, for the 2010 program year. We require three separate and identifiable member outreach efforts. Priority Health will review each request and provide a determination. We’ll notify you if we need additional documentation. We’ve recently updated our non-adherent member exclusion form, which you’ll find at the link below. The Asthma Medication Management measure is no longer offered within this program.
