Help for patients with chronic disease: The PATH Program
Empower your patients: The PATH (Personal Action Towards Health) program, also known as the Stanford Chronic Disease Self-Management Program, is a powerful workshop designed for people with ongoing health conditions like heart or lung disease, diabetes, and arthritis. This evidence-based self-management workshop helps participants learn the techniques and strategies they need — such as goal setting and action plans — to take an active role in managing their health. It is offered at little to no cost to participants and is located at churches, senior centers and community centers across Michigan.
The workshop is led by two leaders, trained and certified by national and state master leaders. Topics covered include stress management, medication use, exercise alternatives, coping techniques and more. Sessions are 2.5 hours, once a week for six weeks, with 10–15 participants per workshop.
