Congratulations to practices recognized by NCQA PPC®-PCMHTM

Nine network practices have received the National Committee for Quality Assurance (NCQA) Physician Practice Connections (PPC) Patient-Centered Medical Home (PCMH) recognition. Congratulations to:

What is the NCQA PPC-PCMH program?

NCQA offers PPC-PCMH recognition to practices completing a program application. There are nine PPC standards with 10 "must pass" elements, which can result in one of three levels of recognition. Practices seeking PPC-PCMH recognition complete a web-based data collection tool and provide documentation that validates responses.

In addition, since Priority Health is a sponsor of the NCQA PPC-PCMH program, practices identifying themselves as part of our network may receive a 20% discount on their application fee if a practice has 15 or fewer physicians and sponsor has 10 or more applications in a market area within a 12-month period.
