Three individual plans available

We now offer three individual plans:
  • MyPriority PPOSM — Complete, affordable coverage for adults, families & children. Includes preventive coverage.*
  • MyPriority U31SM — Individual basic coverage created especially for 18-30-year-olds.*
  • MyPriority Short-termSM — Basic one- to six-month coverage for those who are changing jobs or are recently unemployed. It's geared toward both individuals and their families.*

*Applicants must meet health criteria in these underwritten plans.

Identifying MyPriority members

Members of a MyPriority plan will have an ID card that will indicate which plan they have — PPO, U31 or Short-term. Rider information will not be reflected on the ID card. Log in and verify coverage through the Member Inquiry tool. Or, call the Provider Helpline at 800 942-4765. Members do not select a PCP.

Formulary for MyPriority plans

MyPriority PPO and U31 members have prescription coverage as part of their plan. The complete formulary for these members is part of our online formulary.

MyPriority Short-term members do not have prescription coverage, but do receive Priority Health's discounted drug rate for prescriptions filled at a network pharmacy.
