Priority Health updates the formulary up to six times a year (negative changes occur only twice per year) and reviews Pharmaceutical Management Procedures annually. Both are available at priorityhealth.com/member/medicationcenter. Here, you'll find the most up-to-date and current list of formulary medications, prior authorization forms and Pharmaceutical Management Procedures. Print copies are available upon request.
