Free health assessments for Priority Health Medicare plan members

Priority Health is doing free in-home health assessments for a select group of our Medicare Advantage members. These assessments are just one of the many initiatives we're implementing to continue the exceptional service you and your patients — our members — expect from us.
We'll use the data we collect to help us enhance the effectiveness of the care management programs we offer and achieve the Centers for Medicare and Medicaid Services (CMS) 5-star quality rating.
About the assessments
Priority Health is partnering with CenseoHealth, which contracts with fully trained, licensed physicians, to complete the health assessments.
- The physician will gather a comprehensive history and complete a brief physical exam.
- If your patients are selected and choose to participate, we'll send both you and your patient a summary of information gathered.
- The assessment is free.
- The information we collect will not affect patients' coverage.
- If patients have health concerns, we'll direct them to talk with their primary care provider. We won't interfere with any care your patients are receiving from you.
