Earn financial support for your PCMH recognition through NCQA

Priority Health is continuing our commitment to paying a 12-month infrastructure support fee to new practices successful in their application to NCQA for Physician Practice Connections (PPC) -PCMH
TM recognition.
- The amount of the 12-month payment will correspond with the level of NCQA recognition.
- The payment will only apply to those practices who submit their PPC-PCMHTM survey to NCQA prior to November 1, 2010.
Get a 20% discount on the application fee
We encourage practices to apply for recognition in 2010. Priority Health is an NCQA PPC-PCMH
TM sponsor. Practices identifying themselves as providers in our network will get a 20% discount on their survey submission fee.
