Health is pleased to announce that it has signed a preferred agreement
with Physical Therapy Provider Network (PTPN) – Great Lakes. PTPN
providers will offer physical therapy services to members in southeast
Michigan.
PTPN – Great Lakes network adds 42 clinics to our physical therapy network that now totals 83 clinics in southeast Michigan.

find a physical therapy provider, go to the online Find a Doctor tool on this website. Choose the Advanced Search option, click the Type of Care drop-down menu, and select "Rehabilitation
Provider."
