Providers are paid
$30 per patient for completing and submitting the
required
information via the website or
$15 per patient for faxing the information.
- Providers will be paid the $30 or $15 even if they simply submit existing
information (gathered within 6 months of the member's effective date)
without seeing the patient.
- Information gathered before the patient became a member of
HealthbyChoice IncentivesSM is valid as long as it meets
the 6-month requirement and is submitted by a participating provider.
- When more than one provider enters information or completes and submits
the qualification form for a patient, in most cases only a single provider
will receive payment. Exceptions will be made on a case-by-case
basis.
- Payments are made quarterly, as soon as possible after each calendar
year quarter ends.
Documentation
Checks will be accompanied by a payment register, with the names of each
patient for whom information was submitted listed with the name of the
submitting provider.
