HealthbyChoice Incentives

HealthbyChoice Incentives^SM (HBC Incentives) is a plan that has two levels: Standard and Choice.

• All members new to HealthbyChoice Incentives start at the Choice level of benefits.
• If members of this plan meet certain health criteria, they will qualify for, or maintain, the lower-cost Choice level.
• If members do not meet the requirements within 90 days of their effective date of coverage, they will automatically be transferred to the Standard benefit level.
• ID cards will indicate that the member has HealthbyChoice Incentives and a list of copayments, Standard or Choice.
