PCMH recognition through NCQA

The National Committee for Quality Assurance (NCQA) offers a Physician Practice Connections (PPC®) -PCMHTM program to recognize practices as patient-centered medical home. There are nine PPC® standards, including 10 "must-pass" elements, which can result in one of three levels of recognition. Practices seeking PPC®-PCMHTM recognition complete a Web-based data collection app and provide documentation that validates their responses.

Application discount code

Priority Health recognizes practices that receive PPC-PCMH recognition and is therefore considered a sponsor by NCQA. Practices identifying themselves as part of the Priority Health network may be eligible to receive a 20% discount on their application fee. Use the code CMIPHA on the application.
