Opening and Closing to New Members

For a primary care provider to open or close his or her practice to new Priority Health members of any or all of our plans, send written notice to your Provider Account Executive or Field Service Representative indicating:
  • The date the closing or opening will become effective
  • The plan or plans to which your practice is opening or closing to new members

Provider Change Forms

You may use the Provider Change Form (29KB PDF) or the Behavioral Health Provider Change Form (117KB DOC) to notify us.
 
Effective date
  • We require 60 days advance notice of closing to new patients
  • Opening a closed practice to new patients can be made effective immediately when we receive your written request.

Established patients switching to Priority Health plans
When your practice is closed to new members and one of your patients covered by other insurance then changes insurance to a Priority Health plan, he or she is considered to be an established patient and may designate you as PCP.
