Credentialing application process

Behavioral Health providers only:

• Complete the Preliminary Provider Information Form (131KB PDF)
• Submit it to the address on the form. Priority Health will assess our need for providers with your specialities and will let you know if we can accept a CAQH application.
• Review the Behavioral Health provider participation criteria.
• Open and print a copy of the Priority Health Addendum to the MAHPP Application (120KB DOC).

All providers:

1. Open and print a copy of the Michigan Association of Health Plans Provider Application (585KB DOC)
   OR
   If you are already registered with CAQH, complete this authorization for us to access your online application (82KB PDF).
2. Complete the application (and addendum, if applicable) and mail to the Priority Health Credentialing Department.
3. It will take Priority Health approximately 8 to 12 weeks to complete the credentialing process. The application, supporting documents and verifications are first reviewed by internal staff for completeness and information is verified.

4. The file is then presented to the Credentialing Committee for review and approved or denied. A report of the action taken is then forwarded to the Quality Integration Committee.
5. Practitioners have the right to review certain information submitted in support of their application and the right to correct erroneous information submitted by another party.
