Adding a Provider to an Existing Practice

If the new provider is already credentialed with Priority Health:
Send a letter to your Physician Account Executive, advising Priority Health of the provider's new practice tax id number, primary location and effective date.


If the new provider is not credentialed with Priority Health:
Follow the steps outlined in the How to Apply section of this manual.
