Moving your practice
When you move your practice, you must supply a new practice address effective immediately. A lapse in availability (see the
office availability standards elsewhere in this manual) will cause Priority Health to assign your patients elsewhere to ensure continuity of care.
- 60 days or more before the change takes effect, complete and print the Provider change form (206KB PDF).
- Fax or mail the form to Priority Health at the address/fax number on the form.
