Leaving the network
Priority Health requires written notice, 90 days in advance, if you are retiring or closing your practice and/or leaving our provider network. Physicians or groups who leave the Priority Health network will not be allowed to re-contract with the plan for 12 months, or until the next contract year, whichever is longer.
Failure to give us 90 days' notice may result in claims denials.
Information Priority Health requires before ending your contract
- Notice must be in writing (you may use the change form below)
- Notice must be given at least 90 days prior to the effective date of termination
- Notice requires the signature of the provider or the office manager
- Notice gives reason for termination (ie., moving out of state).
- If you are a Primary Care Physician, provide in writing the arrangement for transfer of members
- Send your notice to your Priority Health Provider Account Representative or Provider Account Coordinator.

Notifying your patients
- You must notify members and/or their employer groups in writing that you are leaving the Priority Health network at least 30 days prior to the date of termination.
- Send us a copy of the letter you sent your patients, and include information on where you would like your members transferred.
