Patient discharges
Under certain circumstances, Priority Health allows PCPs to request member discharges.
Letters notifying patients of discharge
A primary care physician must notify a patient of the intention to discharge him or her in writing on company letterhead. Information should include:
- The reasons for the discharge related to the criteria in the Discharge Policy
- The patient's Priority Health contract ID number
- The patient's date of birth

- The patient must designate a new PCP using the online or PDF PCP change form.

- Send the patient a letter notifying him/her of the discharge; see guidelines above this chart.
- Submit a completed Patient Discharge Form to Priority Health
- Include a copy of the discharge letter sent to the patient.
