Submit it to the address on the form. Priority Health will assess our need for providers with your specialities and will let you know if we can accept a CAQH application.

Complete the application (and addendum, if applicable) and mail to the
Priority Health Credentialing Department
.
The application, supporting documents and verifications are reviewed by internal staff for completeness and information is verified. Our goal is to complete this credentialing process in 45 days or less.

The file is then presented to the Credentialing Committee for review and approved or denied. A report of the action taken is then forwarded to the Quality Integration Committee.
Practitioners have the right to review certain information submitted in support of their application and the right to correct erroneous information submitted by another party.
